LS/CMI Assessment Review (AR) Forms: This form is used to assess the inter-rater reliability for sections 1 through 8 of the LS/CMI.  All reviews should be conducted independently and feedback should be provided to the Reviewed User.

Annual Relationship Skills Assessment Form for Supervisors:  This form is used by LS/CMI User or User Trainer Certified Supervisors to review their subordinates annually on their use of relationship skills with clients.
